The Foreigner Physical Examination Form (also called the International Traveler’s Health Examination Record) is an official Chinese government health document that certifies you are medically fit to study in China. It is required for:

Foreigner Physical Examination Form
Official form issued by China’s National Immigration Administration. This is the standard form accepted for all scholarship and visa applications.

Local Hospital Forms or Generic Health Certificates
Any local hospital health certificate or generic medical form is NOT accepted. You must use the official Chinese government format specifically.

The physical examination form requires several medical tests to be completed by a licensed doctor. Here is exactly what is tested:

Where to Get the Physical Examination Done
How to Fill the Physical Examination Form — Step by Step
Download the Correct Official Form
Get the official Foreigner Physical Examination Form from your target university’s international office website or from Scholarship Navigator. Print it on A4 paper — do not resize.
Fill the Personal Information Section Yourself
Fill in your name, passport number, nationality, and date of birth in the top section exactly as they appear in your passport. Paste a recent passport photo in the photo box. Sign where required.
Take the Form to the Hospital
Bring the printed form to the hospital. The doctor and lab technicians complete the medical sections — never fill in the medical results yourself. The doctor must sign each section and the hospital must stamp the form with an official seal.
Attach Lab Reports to the Form
Attach all lab test results — blood test report, chest X-ray film and report, HIV test result, Hepatitis B & C results, and any other test reports — as supporting documents behind the main form.
Make Certified Copies Before Submitting
Make at least 3 certified copies of the completed form before submitting. You will need the original for your scholarship application, one copy for the visa application, and one to keep for your records.

Important — Validity Period of the Form
⚠️ 6-Month Validity Rule — Do Not Ignore This
The Physical Examination Form is only valid for 6 months from the date of examination. This is one of the most common mistakes students make — they complete the exam early and by the time their scholarship results come out and visa processing begins, the form has expired.
📅 Timing Recommendation:
- If applying for CSC (deadline April) — get the exam done in March or April
- If results come in June–August — the form done in April is still valid ✅
- If you do it in November–December — it expires before your visa appointment ❌

6 Common Mistakes That Get Physical Examination Forms Rejected
Using a Generic Local Health Certificate
Only the official Chinese government Foreigner Physical Examination Form is accepted. Any other format is automatically rejected.
Missing Hospital Stamp or Doctor Signature
Every section must have the doctor’s signature and the hospital’s official stamp. Missing even one stamp means rejection.
Name or Passport Number Mismatch
The name and passport number on the physical form must exactly match your passport. Even a spelling difference causes rejection at the embassy.
Expired Form at Time of Visa Application
Getting the exam done too early — more than 6 months before your visa appointment — means you have to redo the entire examination.
Missing or Incomplete Lab Reports
All supporting lab reports — especially chest X-ray film, HIV result, and Hepatitis B result — must be attached. Missing reports = incomplete application.
No Photo or Wrong Photo Format
The form requires a recent passport-size photo with white background, pasted in the designated box. Missing or wrong format photo is a common rejection reason.
